Dear Patients, 

I am writing to inform you that I will be departing from my practice at TPMG Independence Family Medicine effective May 31, 2026. My decision to leave TPMG was not made lightly, especially given the close relationships I have developed with my patients over the years. But after careful consideration, I decided that it was in my best personal and professional interest to make this move. It has been my distinct honor and pleasure to serve as your family physician. 

Ensuring the continuity of your care is a top priority for both my staff and me. I am pleased to share that at the time of my departure, the new managing physician at TPMG Independence Family Medicine will be Matthew Balderston, MD. Together with Mary L. Huntley-Wagner, FNP-BC and Katie L. Sexton, PA-C, Dr. Balderston will continue to provide you with the high quality, compassionate medical care you deserve. 

To make the transition as smooth as possible, your electronic medical records will remain within the TPMG system; however, if you choose to transition your care outside of TPMG, you may contact the TPMG Medical Records Department at (757) 232-8840 to obtain the necessary paperwork to release your medical records to another provider.
