You should expect a long-term relationship that includes trust, treatment, and accessibility.

Having a primary care provider is essential to your health. Primary care providers will guide you in making the best, informed healthcare decisions. They learn everything about you – your medical history and your family medical history and start treatment from there.
Do I need a referral from my primary care provider to see a specialist?
Your TPMG primary care provider knows you and your medical history and will help you chose the best provider out there for your individual needs.
What is the difference between family medicine, internal medicine, and geriatric medicine?
- A family medicine physician treats children and adults.
- An internal medicine physician treats adults.
- A geriatric medicine physician treats the elderly.
How can I contact my primary care provider?
Are you accepting new patients?
While many of our primary care practices are accepting new patients, some are not. Please contact your future primary care office’s webpage to determine if the practice is accepting new patients or utilize the “Find A Provider” toolbar, select the “Primary Care” specialty, and click the box marked “Accepting New Patients” to find a primary care provider nearest you.Â

New Patients:
- All medications you are currently taking (include prescription, over-the-counter, and herbal remedies)
- Your current health insurance card
- Photo identification
- A referral from your primary care physician, if required by your insurance plan
- A list of allergies to medications or dyes
- Immunization record for children
- Payment for any required co-pays, deductibles, or non-covered services
