We are excited to see our former patients and welcome them to schedule an appointment with us at our new location in Williamsburg at New Town. We offer and encourage our patients to use the TPMG Patient Portal for electronic communications, such as requesting appointments and refills, communicating with us, or paying their bills online. Our staff has privileges at Riverside Doctors’ Hospital Williamsburg and Sentara Williamsburg Regional and access to EPIC Health Records for added convenience to your pertinent healthcare information. Dr. Glenn Rauchwarg and Dr. Thomas Hoag have been practicing in the Williamsburg area since 1998 and 2005, respectively. Our primary care clinic at TPMG Discovery Park Family Medicine is passionate about preventative medicine since many serious health conditions can be avoided with prior intervention. We recommend patients receive annual comprehensive physicals to evaluate acute and chronic concerns. In addition, patients should remain up to date with routine screening testing and immunizations. As illnesses and diseases evolve, so does our preventative practice. TPMG Discovery Park Family Medicine focuses not only on managing acute and chronic health concerns but also routinely reviews evidence-based disease prevention screenings. Our providers will then make recommendations including in-network referrals, testing, and education. We believe in the importance of open, provider-patient dialogue and encourage our patients to discuss any signs or symptoms that may be related to an undiagnosed medical condition. Through communication, we can lead our patients to better health outcomes. TPMG Discovery Park Family Medicine advocates for assisting patients by improving their overall health and helping them achieve their wellness goals. This involves weight loss counseling and dietician referrals, in-office testing (Strep, Flu A/B, COVID-19, UTI, etc.), TB testing, and routine vaccinations (including recommended childhood vaccinations). *Please note, booking appointments online are available for Barbara RoutĂ©, FNP-C, Lauren Reynolds, FNP-C, and Amanda Schaeffer, PA-C only.  To provide additional support between appointments, TPMG participates in Medicare’s Advanced Primary Care Management (APCM) program.
