Medicare’s Advanced Primary Care Management (APCM) program helps your primary care team coordinate and personalize your care between visits. From preventive care planning and medication reviews to chronic condition support and follow-up after hospital stays, APCM keeps your care team working for you all month long. Enrollment is voluntary, you keep your same providers, and you can cancel at any time. Watch Dr. Glenn Rauchwarg of TPMG Discovery Park Family Medicine explain how APCM works, what it costs, and whether it’s right for you.

Why Choose a Primary Clinician on MyMedicare.gov?
Your primary clinician is a health care professional – doctor, physician assistant, or nurse practitioner – who’s responsible for coordinating your overall care, regardless of where you choose to get services. He/She may be able to help you make health care decisions, which can improve how you manage your health care and lead to better results.
By choosing a primary clinician on MyMedicare.gov, you give your physician access to tools or services that are only available to patients of healthcare professionals participating in an Accountable Care Organization (ACO) or other Medicare alternative payment models. ACOs are groups of doctors, hospitals, and other healthcare professionals working together to provide you with high-quality, coordinated healthcare.
Patient Benefits for Using MyMedicare.gov
You have the option to choose any professional as your primary clinician, regardless of specialty, whether it’s your TPMG Primary Care Physician, allergist, immunologist, nurse practitioner, physician assistant, etc. You’re able to choose ANY healthcare professional who accepts Medicare.
Medicare offers an Annual Wellness Exam for those who have been enrolled in Medicare for more than 12 months. The annual wellness exam is at no cost to you unless other acute/chronic conditions are present (e.g., colds, injuries, high blood pressure, diabetes, etc.) and should not be confused with an annual physical exam. Click here to learn more.
Lastly, you may choose another healthcare professional as your primary clinician at any time. Your choice of a primary clinician will remain the same unless you make a change on MyMedicare.gov.
Here are instructions to help you choose a primary care physician on MyMedicare.gov.
Voluntary Alignment
Medicare beneficiaries can voluntarily align with their primary care provider on MyMedicare.gov. Voluntary alignment increases patient-centered care by prioritizing patient choice when attributing patients to a practice and better engages beneficiaries in their health care. The Centers for Medicare & Medicaid Services (CMS) uses beneficiaries’ attestations as the prioritized method of attribution for practices participating in the TPMG model of care. Here is more information about Medicare Voluntary Alignment.
