Your electronic medical records will remain within the TPMG system; however, if you wish to transition your care outside of TPMG, you may contact our office or the TPMG Medical Records Department at (757) 232-8840 to obtain the necessary paperwork to release your medical records to another provider. We also have a downloadable option and a link to online submission for records release on our website at: https://mytpmg.com/patient-info/release-of-information/. Â
