However, if you are experiencing a medical emergency, please call 911 or visit your nearest emergency room immediately. Our lab services are no longer available in office, however, our staff will provide a slip for our patients to receive lab services at a location of their choosing. We apologize for the inconvenience and appreciate your patience. New Patients – The New Patient Paperwork MUST be turned in one week before your scheduled appointment. Failure to do so will result in rescheduling/cancelling your appointment. Forms can be dropped off in our office, faxed, or uploaded to the patient portal. No-Show Policy strictly enforced – please contact the office at least 24 hours in advance if you are unable to keep your scheduled appointment. Failure to do so will result in no-show fees, which are not covered by medical insurance. To provide additional support between appointments, TPMG participates in Medicare’s Advanced Primary Care Management (APCM) program.
